Bitfinex today was hacked and lost 120,000 in their multisig bitcoin addresses held by customers. Zane, PR for Bitfinex, has confirmed this on reddit:
Speculation had been spreading, as individuals noticed in the mempool as well as the outstanding multisig balances indicated thousands of bitcoin being moved on the blockchain:

Bitfinex does not use cold wallet security procedures, they just use multisignature BitGo wallet provision. This may indicate the weakness of using this kind of security model as an exchange.
It is unclear how Bitfinex will cover this gigantic loss, but their current statement shows that BTC holders are the only ones affected.
